Initial modal window with title bar only

EDP, version 12.0.0.6
Just started to happen and I cannot figure out how to correct this problem. When I bringup ExamDiff Pro, the initial modal window to fill in the two paths is not shown. The only thing shown in this modal window is the title bar. Nothing I do can change it. I have uninstalled and reinstalled EDP, and still no change. Not an issue with any other application.
Is there a way to uninstall so that all configuration is wiped? (i.e., are there registry keys that can be removed to fix this...)
